What documents do I need to submit with my application?

You must give us the documents asked for in the lodgement guide (see pages 2–4 and page 15)—part of the grant application (PDF 750 K). The types of documents we need depend on your particular circumstances.

You can use the table below as a guide. For more information see pages 2–4 of the lodgement guide.

Proof of identity

All applicants and their spouse/de facto partners must provide a category 1 document as proof of identity.

Category 1 document Comments

Persons born in Australia:

  • Australian birth certificate/extract
  • Current Australian Passport

 

Persons born overseas:

  • Australian citizenship certificate
  • Current passport with proof of permanent visa or special category visa
  • Certificate of residence issued by the Department of Immigration
  • At least one applicant must be a permanent resident or citizen of Australia at the date of application.
  • Non-applicant spouse/de facto partners do not need to give evidence of a visa or citizenship, however, they still need to provide a current passport or birth certificate.
  • New Zealand citizens must be living in Australia at the date of application.

If you apply through OSR, you also need to give us a document from each of the following categories. You need to provide a separate document for each category. The documents should be current.

Category 2 document
  • Australian Driver Licence
  • Passport
  • Firearms licence
  • 18+ Card
Category 3 document
  • Medicare card
  • Motor vehicle registration
  • Concession or Department of Veterans Affairs card
  • Debit/credit card of a lending institution
Category 4 document
  • Utility documents showing current residential address
  • Insurance policy showing current residential address
  • Statement of account with a lending institution showing current residential address 

 

Certification of documents

All proof of identity documents must be certified copies. Documents may be certified by a:

  • Justice of the Peace
  • Commissioner for Declarations
  • notary public.

We will also accept certification by a/an:

  • lawyer
  • conveyancer
  • magistrate
  • police officer
  • dentist
  • pharmacist
  • certified practicing accountant
  • Member of Parliament
  • medical practitioner
  • Office of State Revenue employee
  • employee of an approved agent (not a mortgage broker).

If you use one of the individuals in the second list to certify your copies, they must:

  • state that they have sighted the original and it is a true copy
  • detail their name and occupation

    and

  • sign and date the copy.

Note: Documents that are not in English must be accompanied by a certified English translation. 

Additional supporting evidence

Depending on your circumstances you may also need to provide additional supporting evidence with your application.

If you are: Please provide:
married a certified copy of your marriage certificate
divorced a certified copy of the Decree Nisi
widowed a certified copy of the death certificate of your spouse/de facto partner
separated

a statutory declaration with the following information:

  • the name of your former spouse/de facto partner
  • spouse/de facto partner’s date of birth
  • date of marriage or commencement of domestic relationship
  • date of separation
  • former spouse/de facto partner’s current address (if known)
  • whether or not you currently reside with your former spouse/de facto partner and whether or not you have the intention to resume cohabitation.

Where you have changed your name for reasons other than those outlined above, you must provide certified copies of evidence of each name change, for example, a change of name certificate or deed poll. Alternatively, please provide a statutory declaration detailing each name you have been known by.

Note: Documents that are not in English must be accompanied by a certified English translation.

How long will it take for my application to be processed?

We will process most applications within 10 working days of receiving all required information.

If you apply through an approved bank or lending institution, you will need to check with them how long it will take them to process the application.

What can I do if my application is not approved?

If you are dissatisfied with a decision regarding your application for a grant, you may lodge a written objection with the commissioner. You must state the grounds of your objection in full and lodge the objection within 60 days of receiving notice of the decision.

Where can I get a registration confirmation statement or current title search?

The Department of Environment and Resource Management issues registration confirmation statements and titles searches.

I am a New Zealand citizen and my passport has expired. What other documentation can I submit?

If you do not have a copy of a current passport, you may submit a Certificate of Residence from the Department of Immigration.

I have separated from my spouse. What documentation do I need to submit to explain why they are not included on my application?

You need to provide a statement outlining your relationship with your former spouse.
